C.I.S.S. Survey-- opportunity for input!

FV Indiana urges our members to take a few minutes to complete the following survey. It is a great opportunity to share your input!

The Indiana State Department of Health needs your feedback to help improve access to quality, comprehensive, coordinated, community-based systems of services for Children and Youth with Special Health Care Needs and their families. The goal is for these systems to also be family-centered, community-based and culturally competent. Please take 10 minutes to complete the Survey; your feedback is extremely valuable to us. Thank You.

You can click this link and access the survey. http://www.surveymonkey.com/s.aspx?sm=1rt_2flVECE_2f98RL_2barHHQYg_3d_3d

There will be paper copies of the Survey available for families at ASK (1-800-964-4746) and the Children’s Special Health Care Services Office at Riley Hospital (317-274-3755).

You can view that DOE powerpoint here.

You may note references to ISTART7. ISTART7 stands for Indiana Standards Tool for Article 7 Compliance. It is the electronic IEP that has been developed by the Indiana Department of Education, Division of Exceptional Learners in collaboration with the ICAN Project and ICAN partnerships across the state.
